| UMDNJ STUDENT HEALTH INSURANCE - UNIVERSITY HEALTH PLANS |
Under University Policy, all full-time UMDNJ students, as well as those part-time UMDNJ students who participate in clinical experience as part of their educational programs, are required to be covered by Health and Accident Insurance. UMDNJ has selected a plan offered by Aetna Life Insurance Company.
The Plan offers two levels of benefits: an "in-network" benefit and an "out-of-network" benefit. It is an 'Open Choice PPO Plan' so students are no longer required to select a Primary Care Physician. To find a provider, log onto http://www.aetna.com/docfind, select the appropriate provider category and select 'Open Choice PPO' from the'Select a Health Plan' drop down menu. To receive the highest level of insurance benefit, simply choose an Aetna participating provider.
All full-time students and those part-time students who participate in clinical experience and pay tuition directly to UMDNJ will be automatically enrolled in and billed for the Student Health Insurance Plan on their UMDNJ tuition bill. They do not need to complete an Enrollment Form. However, if those students wish to enroll eligible dependents, they must complete a DEPENDENT ENROLLMENT FORM. This Form will be available at the: UMDNJ UniversityHealthPlans web site. The completed Form and premium must be sent to our service provider, University Health Plans, Inc., One Satterymarch Park, Quincy, MA 02169. The deadline for enrolling dependents is August 15, 2002. Additional information about the plan, including pharmacy prescriptions, also are located at this site.
The individual coverage premium for GSBS doctoral students receiving a GSBS stipend will be paid by the graduate school or the student's mentor.
Students requesting to waive participation in the Student Health Insurance Plan must complete a Waiver Form that indicates they have comparable coverage from another source. The on-line waiver option allows students to quickly and easily waive participation in the Plan. Select 'Waver Form" from the drop down menu.
